Principles of Care Co ordination

A number of principles underpin an effective care coordination program:

  1. Person-centred care – Person-centred-care must form the basis of care coordination
  2. Communication – Stakeholders in the client’s health and wellbeing must be engaged with each other and the care coordinator
  3. Access and equity – A care coordination program must be accessible and equitable
  4. Accountability – A care coordinator must be accountable
  5. Quality and safety – A care coordinator must be safe and provide high quality service
  6. Confidentiality – Client information must be kept confidential and private
  7. Wellness – An effective care coordinator will encourage wellness
